What happens to items a person has to leave behind at the airport due to overweight luggage?

How do they know if you have drugs in your carry on luggage when you fly?

Cause I know they probably have sniffer dogs behind the scenes to sniff your checked in bag. But how would they know if a pot of medication in your hand luggage arent illegal drugs?
